A Clinical Observation of Traumatic Injuries of Duodenum and Pancreas



Abstract
This report presents clinical findings of the 31 cases of traumatic pancreatico-duodenal injuries who had been treated for about 14 years from 1970 to 1983 at the department of general surgery in National Medical Center. The results were summarized briefly as follows.
1) The incidence of pancreatic and duodenal injuries was 31 cases(13%) of injured patients (245 cases) by abdominal trauma.
2) The age of the 31 patients ranged from 7 to 55 years: 29 patients between 10 and 49 years of age. Sex ratio was 30(male): 1(female).
3) The cause of injuries was blunt injuries (25 cases): 12 cases by violence, 10 cases by traffic accident and 6 cases by penetrating injuries. All cases of penetrating injuries were happened by knife.
4) Clinical diagnosis was more difficult than other type of abdominal injuries and clinical confirmative diagnosis was only 5 cases. (16%)
5) Among the 31 cases, pancreatic injury was 15 cases, duodenal injury 13 cases (11 cases of retroperitoneal rupture, 2 cases of intramural hematoma) and combined pancreatico-duodenal injury 3 cases.
6) Post-operative complications were as follows:
i) Among the 15 cases of pancreatic injury, post-operative complications were 8 cases (53%): 4 cases were pancreatic fistula, 2 cases pancreatic pseudocyst and 2 cases pan-creatitis. No mortality was found.
ii) Among the 13 cases of duodenal injury, post-operative complications were 8 cases of infection, 6 cases of duodenal fistula, 2 cases of lung complication, 1 case of renal failure and 1 case of gastrointestinal hemorrhage. Post-operative deaths were 2 cases. (15%)
iii) All 3 cases of combined pancreatico-duodenal injuries were died postoperatively: 1 case of hemorrhage and 3 cases of multiple organ failure due to sepsis.
